Community Support

The primary goal of the Community Support Department is to be able to offer direct support to families and individuals in a consumer/family focused manner.

Early Support & Services serves children from birth to three years of age whose development is delayed. Early Support & Services are family focused and are provided in the families homes through one of our two vendor agencies.

Family Support

Family Support offers a variety of support options for families. Family Support provides the following:

  • Information and Referral - for all families, regardless of disability
  • Home Visits - from Family Support staff or support team member
  • Respite Care - respite is designed to offer families a temporary break in providing care for individuals who have developmental disability, and who live with their families
  • Parent to Parent Programs - is a regional network that connects parents for supportand information
  • Exploring funding options for - medical/dental costs, home modifications, adaptive equipment, special food/clothing and other special needs
  • Referrals for consultations for families on - behavioral, dietary and other pertinent issues
  • Information about and assistance - with transition from school to the adult system
  • Training and Information Sessions
  • Camperships

Support Coordination

Support Coordination acts to coordinate a variety of support to adults with disabilities and their families including, but not limited to, the following:

  • Information and referral
  • Connecting to generic community services
  • Assistance in applying for benefits such as Social Security, Medicaid and APTD
  • Assistance in exploring guardianship alternatives
  • Advocate for the needs of individuals on the waiting list

Support Coordinators act as advocates and monitor services (i.e. day program, vocational program, personal care services, and community support). In these situations individuals with disabilities are assisted in developing an annual plan which identifies the areas where they are requesting support.
